Service to Improve Access
What are the current 'Services to Improve Access' initiatives?
The PHO currently runs the following projects under the Service to Improve Access Programme:
All services under the Service Improve Access programme are targeted at Maori, Pacific Island and Dep9-10 enrolled patients.

PURPOSE

Free Health Checks
|
Any person who resides in the Takitimu PHO area and is not enrolled at a Takitimu PHO medical clinic is entitled to a free health check and follow-up on enrolment with their chosen medical clinic.

Transport Vouchers
|
Patients who would otherwise be unable to attend medical appointments as they are unable to afford transport may access transport assistance through their local community worker.
|
|
Care Vouchers
|
Patients who have not attended General practice in the last 6 months, require immediate access to the GP and would otherwise be unable to due to their financial circumstances, may get a one-off Care Voucher through their local community worker.

|
Warm Homes
|
Funding is provided where the person/family meets the Warm Homes low income criteria and the person suffers from a respiratory condition
